Parents can book their child in for an initial assessment with one of our therapists for advice and recommendations on NDIS eligibility. We can also provide supporting documents, reports and letters of referral to assist in your application for the NDIS.

Anyone who thinks they might be eligible for the NDIS can download an Access Request Form (ARF), and a Supporting Evidence Form (SEF), work with their GP, allied health, and specialist medical professionals to prepare and provide the information required.
